Diagnosis & Investigation Pathway

Primary Diagnostic Tools

  • X-ray of affected bone (initial test at most centres)
  • MRI of the involved limb or region
  • CT scan of the affected bone and chest (for metastasis)
  • Bone scan (where available)
  • Core needle or open biopsy (orthopaedic oncology or surgical team)

Pakistan-Specific Notes

  • X-ray is usually first-line due to accessibility
  • MRI available at tertiary centres and major cities
  • Bone scan limited to larger nuclear medicine centres
  • Biopsy should be planned by an orthopaedic oncologist to avoid compromising future surgery

Treatment Pathway (Pakistan-Adjusted)

Localised High-Grade Bone Sarcoma (e.g., Osteosarcoma)

  • Neoadjuvant chemotherapy
  • Limb-salvage surgery where possible (or amputation if necessary)
  • Adjuvant chemotherapy

Metastatic Bone Sarcoma

  • Systemic chemotherapy
  • Palliative radiotherapy for pain control
  • Orthopaedic stabilisation for pathological fractures

Pakistan Resource Notes

  • Chemotherapy available at major cancer centres
  • Limb-salvage surgery limited to specialised orthopaedic units
  • Radiotherapy available at tertiary centres
