Betel Nut, Gutka, and Paan: A Hidden Cancer Epidemic

This article has been reviewed, edited and approved by Dr Fakhra Qureshi, Consultant Oncologist, Rawalpindi, Pakistan.

In Pakistan and South Asia, chewing betel nut (supari), gutka, and paan is deeply woven into daily life. It is offered at gatherings, shared among friends, and even given to children. But behind this cultural habit lies a devastating health crisis: these products are among the most powerful causes of oral cancer in the world.

What are these products?

  1. Paan (بیڑہ): A mixture of betel leaf, betel nut (supari), slaked lime (chuna), and sometimes tobacco, wrapped into a quid and chewed.

  2. Gutka (گٹکا): A commercially packaged mixture of betel nut, tobacco, slaked lime, and flavourings — sold in small sachets at every paan shop.

  3. Supari (سپاری): Betel nut on its own, often chewed as a stimulant. Even without tobacco, betel nut itself is carcinogenic.

  4. Naswar (نسوار): A smokeless tobacco product placed under the lip, common in Khyber Pakhtunkhwa and northern Pakistan.

Why is this so dangerous?

The World Health Organization (WHO) classifies betel nut (areca nut) as a Group 1 carcinogen — the same category as tobacco and asbestos. This means there is clear, proven evidence that it causes cancer in humans.

When you chew these products, the harmful chemicals stay in constant contact with the inside of your mouth — the cheeks, gums, and tongue — for long periods. Over time, this causes:

  • Oral submucous fibrosis (OSMF): A condition where the mouth becomes stiff and the cheeks lose flexibility, making it difficult to open the mouth. OSMF is a pre-cancerous condition — it can progress to cancer.
  • Leukoplakia: White patches in the mouth that can turn cancerous.
  • Oral cancer: Tumours on the tongue, cheek, gums, or floor of the mouth.

The scale of the problem in Pakistan:

  • Pakistan has one of the highest rates of oral cancer in the world.
  • In many regions, 10–37% of people chew paan or gutka.
  • Oral cancer is often diagnosed late because the early signs — white patches, mouth stiffness, mild pain — are ignored.
  • Gutka is cheap, easily available, and aggressively marketed, even to children.

Warning signs of oral cancer:

  • A mouth ulcer that does not heal within 2–3 weeks
  • White or red patches inside the mouth
  • Difficulty or pain when opening the mouth
  • A lump or thickening in the cheek
  • Difficulty swallowing
  • Loose teeth without an obvious cause
  • Bleeding in the mouth

If you notice any of these signs, see a dentist or doctor immediately. Early detection of oral cancer saves lives.

Common myths:

  • "Paan without tobacco is safe." — False. Betel nut alone causes cancer, even without tobacco.
  • "Gutka only stains teeth." — False. The damage goes far deeper, causing irreversible changes in the mouth tissue.
  • "Only old people get oral cancer." — False. Young gutka users are being diagnosed with oral cancer in their 20s and 30s.

What should you do?

  • Stop chewing betel nut, gutka, paan, and naswar completely.
  • If you have been chewing for years, see a dentist for an oral cancer screening — it is quick, painless, and could save your life.
  • Do not give these products to children.
  • Encourage family members who chew to stop.

Key message: Betel nut, gutka, and paan are not harmless cultural habits — they are proven causes of cancer. Pakistan's oral cancer rates are among the highest in the world because of these products. Stopping today reduces your risk, and early screening can catch problems before they become cancer.

(Leukoplakia)
