Stomach Cancer: The Silent Disease

This article has been reviewed, edited and approved by Dr Fakhra Qureshi, Consultant Oncologist, Rawalpindi, Pakistan.

Stomach (gastric) cancer is often called a "silent disease" because it rarely causes symptoms in its early stages. By the time most people in Pakistan realise something is wrong, the cancer has often already spread. Understanding the warning signs and risk factors can help catch stomach cancer earlier — when treatment is more likely to succeed.

What is stomach cancer? Stomach cancer begins when cells in the lining of the stomach start growing abnormally. Over time, these cells can form a tumour. Stomach cancer often develops slowly over many years.

Why is stomach cancer "silent"? The stomach is a large, flexible organ. A tumour can grow inside it for a long time without causing pain or obvious symptoms. Early symptoms are vague and easily mistaken for common digestive problems — which nearly everyone has from time to time.

Early symptoms that are often ignored:

  • Indigestion or heartburn that doesn't go away
  • Feeling full after eating small amounts (early satiety)
  • Mild nausea
  • Loss of appetite
  • Bloating after meals

These symptoms are so common that most people dismiss them as "just gas" or acidity. But when these symptoms persist for more than 2–3 weeks without an obvious cause, they should be investigated.

More advanced symptoms:

  • Unexplained weight loss
  • Persistent abdominal pain (usually in the upper abdomen)
  • Vomiting, sometimes with blood
  • Difficulty swallowing
  • Black, tarry stools (a sign of bleeding in the stomach)
  • Fatigue (from anaemia caused by slow bleeding)
  • A lump in the abdomen

Risk factors in Pakistan:

  • H. pylori infection: A common bacterial infection of the stomach that increases cancer risk. It is widespread in Pakistan.
  • Diet: High intake of salted, smoked, and pickled foods. Low intake of fresh fruits and vegetables.
  • Smoking: Significantly increases risk.
  • Family history: Having a close relative with stomach cancer.
  • Age: More common after 50.
  • Chronic gastritis: Long-term inflammation of the stomach.

What to do if you have persistent symptoms:

  1. See a doctor — don't dismiss persistent indigestion as "just gas."
  2. Request an endoscopy (OGD — oesophagogastroduodenoscopy). A thin, flexible tube with a camera is passed through the mouth into the stomach. This is the best way to see stomach cancer early.
  3. A biopsy may be taken during the endoscopy to check for cancer cells.

In Pakistan, endoscopy is available at all major teaching hospitals and private hospitals. It is a safe, quick procedure done under sedation.

The problem in Pakistan:

  • Persistent indigestion is extremely common and usually dismissed.
  • Many people self-medicate with antacids for months or years before seeking help.
  • By the time stomach cancer is diagnosed, it is often at an advanced stage.
  • Endoscopy is underutilised — many people are afraid of it, but it is quick and painless under sedation.

Prevention:

  • Treat H. pylori infection if diagnosed.
  • Eat more fresh fruits and vegetables.
  • Reduce salted, smoked, and pickled foods.
  • Don't smoke.
  • Don't ignore persistent digestive symptoms.

Key message: Stomach cancer is a silent disease that hides behind common digestive complaints. If you have indigestion, fullness, or bloating that persists for more than 2–3 weeks, don't dismiss it as "just gas." Ask your doctor for an endoscopy. Early detection of stomach cancer saves lives.
