Always Tired? When Fatigue Is a Warning Sign

This article has been reviewed, edited and approved by Dr Fakhra Qureshi, Consultant Oncologist, Rawalpindi, Pakistan.

Everyone feels tired sometimes — it's part of life. But persistent, unexplained fatigue that doesn't improve with rest can be a warning sign of something more serious, including cancer. Understanding when fatigue is concerning — and when it's simply life — is important.

What is fatigue? Fatigue is more than just feeling sleepy. It's a deep, persistent tiredness that affects your body and mind. You may feel:

  • Exhausted even after a full night's sleep
  • Unable to do your normal activities
  • Weak or heavy in your limbs
  • Unable to concentrate
  • Lacking motivation

When is fatigue normal?

  • After a long day or poor sleep
  • During times of stress
  • After intense exercise
  • During a minor illness (like a cold)
  • During pregnancy

Normal fatigue improves with rest and resolves within days to a week.

When is fatigue a warning sign? Fatigue becomes concerning when it is:

  • Persistent: lasting more than 2–3 weeks
  • Unexplained: not caused by obvious factors like poor sleep or stress
  • Progressive: getting worse over time
  • Interfering: stopping you from doing your normal activities
  • Accompanied by other symptoms: weight loss, fever, night sweats, pain, or bleeding

Cancers where fatigue is a key symptom:

  • Blood cancers (leukaemia, lymphoma): The bone marrow can't produce enough healthy red blood cells, causing anaemia and severe fatigue.
  • Stomach and colorectal cancer: Slow, hidden bleeding leads to iron-deficiency anaemia, causing fatigue.
  • Thyroid cancer and other thyroid disorders: Hormonal imbalances cause extreme tiredness.
  • Any advanced cancer: The body diverts enormous energy to fighting the disease.

Other symptoms to watch for alongside fatigue:

  • Unexplained weight loss
  • Fever or night sweats
  • Swollen lymph nodes (neck, armpits, groin)
  • Easy bruising or bleeding
  • Pale skin
  • Shortness of breath
  • Persistent pain

Other medical causes of fatigue (not cancer):

  • Anaemia (iron deficiency) — very common in Pakistan, especially in women
  • Thyroid disorders (hypothyroidism)
  • Diabetes
  • Vitamin D deficiency — extremely common in Pakistan
  • Depression and anxiety
  • Chronic infections (tuberculosis, hepatitis)
  • Heart disease
  • Sleep disorders

What should you do?

  1. Don't ignore persistent fatigue. If you've been unusually tired for more than 2–3 weeks, see a doctor.
  2. Get basic blood tests:
    • Complete blood count (CBC) — checks for anaemia, blood cancers
    • Iron levels (ferritin)
    • Thyroid function tests (TSH)
    • Vitamin D levels
    • Blood sugar (diabetes)
    • Liver and kidney function
  3. Be honest with your doctor about all symptoms — even if they seem minor.

The problem in Pakistan:

  • Fatigue is extremely common and usually dismissed as "overwork" or "stress."
  • Many women attribute fatigue to household responsibilities or periods.
  • Self-medication with vitamins and tonics is common — but without testing, the underlying cause is never found.
  • Cancer and other serious conditions are diagnosed late because fatigue is ignored.

Key message: Persistent, unexplained fatigue is not "just life." If you've been unusually tired for more than 2–3 weeks, don't ignore it. A simple blood test can reveal the cause — whether it's iron deficiency, thyroid problems, diabetes, or something more serious like cancer. Don't self-medicate — get tested and find out what's really going on.
