Colonoscopy vs Stool Test: Which One Should You Choose?

This article has been reviewed, edited and approved by Dr Fakhra Qureshi, Consultant Oncologist, Rawalpindi, Pakistan.

Colorectal cancer is becoming increasingly common in Pakistan. The good news is that it is one of the most preventable cancers when caught early through screening. There are two main screening methods: colonoscopy and the stool test (faecal occult blood test). This article explains both so you can make an informed choice.

Why screen for colorectal cancer? Colorectal cancer usually starts as a small, non-cancerous growth called a polyp. Over 5–10 years, some polyps can turn into cancer. If we find and remove polyps during screening, we can prevent cancer entirely — not just catch it early, but actually prevent it.

Who should be screened?

  • All adults aged 45–50 and above
  • Earlier if you have: a family history of colorectal cancer, blood in stool, persistent change in bowel habits, or unexplained weight loss

Option 1: Colonoscopy

What it is: A doctor inserts a thin, flexible tube with a camera (colonoscope) through the anus to examine the entire large bowel (colon). The procedure takes 20–30 minutes.

Advantages:

  • Most effective: It can find AND remove polyps in the same procedure — preventing cancer before it starts.
  • Comprehensive: The entire colon is examined directly.
  • Long interval: If normal, you don't need another one for 10 years.
  • Gold standard: Considered the most accurate screening method.

Disadvantages:

  • Requires bowel preparation (drinking a strong laxative the day before — unpleasant but important)
  • Done under sedation — you'll need someone to take you home
  • Small risk of bleeding or perforation (very rare)
  • More expensive than a stool test
  • Requires a hospital visit

In Pakistan: Available at all major teaching hospitals and private hospitals. The cost varies — government hospitals are cheaper, private hospitals more expensive.

Option 2: Stool Test (Faecal Occult Blood Test — FOBT)

What it is: A simple test that checks a small sample of your stool for hidden blood that you can't see with the naked eye. Blood in the stool can be a sign of polyps or cancer.

Advantages:

  • Simple and easy: You collect a small sample at home.
  • No preparation: No laxatives, no sedation, no hospital visit.
  • Affordable: Much cheaper than a colonoscopy.
  • Widely available: Available at any diagnostic lab in Pakistan.

Disadvantages:

  • Less comprehensive: It only detects blood, not polyps directly.
  • False positives: Blood can come from other causes (haemorrhoids, ulcers) — a positive test doesn't mean cancer.
  • False negatives: Not all polyps or cancers bleed, so some can be missed.
  • Needs repeating: If normal, should be done every year.
  • If positive, you still need a colonoscopy: A positive stool test means you MUST get a colonoscopy to find the source of bleeding.

Which one should you choose?

  • Accuracy: Colonoscopy — Highest; Stool Test — Moderate
  • Can remove polyps: Colonoscopy — Yes; Stool Test — No
  • How often: Colonoscopy — Every 10 years; Stool Test — Every year
  • Preparation: Colonoscopy — Bowel prep needed; Stool Test — None
  • Cost: Colonoscopy — Higher; Stool Test — Lower
  • Hospital visit: Colonoscopy — Yes; Stool Test — No
  • If positive: Colonoscopy — N/A; Stool Test — Needs colonoscopy

The recommendation:

  • If you can afford it and are comfortable with the preparation, colonoscopy is the best choice — it both detects and prevents cancer.
  • If cost or access is a barrier, the annual stool test is a good alternative — it's far better than no screening at all.
  • If your stool test is positive, you MUST get a colonoscopy.

Don't wait for symptoms: Colorectal cancer often has no symptoms in early stages. By the time symptoms appear (blood in stool, weight loss, change in bowel habits), the cancer may already be advanced. Screening finds problems before symptoms develop.

Key message: Both colonoscopy and stool tests save lives. Colonoscopy is the gold standard because it can prevent cancer by removing polyps. The stool test is a simpler, cheaper alternative that is better than nothing. If you are 45 or older, talk to your doctor about which test is right for you. Don't wait for symptoms — screen before cancer develops.
