Cancer Screening in Pakistan: Which Tests You Actually Need

This article has been reviewed, edited and approved by Dr Fakhra Qureshi, Consultant Oncologist, Rawalpindi, Pakistan.

Cancer screening means checking for cancer before you have any symptoms. It is one of the most powerful tools we have to catch cancer early — when it is most treatable. But in Pakistan, screening is still rare, and many people only see a doctor when symptoms are already advanced.

This guide explains which cancer screening tests actually matter, who should get them, and where they are available in Pakistan.

Why does screening matter? When cancer is found early (Stage I or II), treatment is simpler, cheaper, and far more likely to succeed. When cancer is found late (Stage III or IV), treatment becomes harder, more expensive, and survival rates drop significantly. In Pakistan, more than half of all cancers are diagnosed late — largely because people do not get screened.

1. Breast Cancer Screening

Breast cancer is the most common cancer in Pakistani women.

  • Breast self-examination: All women aged 20+ should examine their breasts monthly. Look for lumps, changes in shape, nipple discharge, or skin changes.
  • Clinical breast examination: A doctor examines your breasts. Recommended every 1–3 years for women aged 20–39, and annually from age 40.
  • Mammogram: An X-ray of the breast. Recommended annually for women aged 40 and above, or earlier if there is a family history of breast cancer.

In Pakistan, mammograms are available at major hospitals including Shaukat Khanum, INMOL, Aga Khan Hospital, and most private diagnostic centres.

2. Cervical Cancer Screening

Cervical cancer is preventable through screening and HPV vaccination, yet it remains common in Pakistan.

  • Pap smear: Recommended for women aged 21–65, every 3 years. Cells from the cervix are examined for pre-cancerous changes.
  • HPV test: Checks for the human papillomavirus, which causes cervical cancer. Can be done every 5 years.

In Pakistan, Pap smears are available at gynaecology departments in teaching hospitals and private clinics.

3. Colorectal Cancer Screening

Colorectal cancer is becoming more common in Pakistan.

  • Faecal occult blood test (FOBT): A simple stool test that checks for hidden blood. Recommended annually from age 45–50.
  • Colonoscopy: A camera exam of the bowel. Recommended every 10 years from age 45–50, or earlier if you have a family history or symptoms like blood in stool.

In Pakistan, colonoscopy is available at major teaching hospitals and private hospitals.

4. Liver Cancer Screening

Pakistan has high rates of hepatitis B and C, which are the leading causes of liver cancer here.

  • Liver ultrasound: Recommended every 6 months for anyone with cirrhosis, chronic hepatitis B, or chronic hepatitis C.
  • AFP blood test: A tumour marker for liver cancer, often done alongside the ultrasound.

If you have hepatitis B or C, ask your doctor about regular liver screening.

5. Oral Cancer Screening

Due to high rates of paan, gutka, and naswar use, oral cancer is extremely common in Pakistan.

  • Oral examination: A dentist or doctor examines the inside of the mouth for white patches, ulcers, or lumps. Quick, painless, and should be done annually — especially if you use betel nut, gutka, or tobacco.

6. Prostate Cancer Screening

For men, prostate cancer screening may be considered from age 50 (or earlier with family history):

  • PSA blood test: Measures prostate-specific antigen levels.
  • Digital rectal examination: A doctor examines the prostate.

Discuss with your doctor whether screening is right for you.

Who should get screened?

  • Breast — Women 40+ — Annual mammogram
  • Cervical — Women 21–65 — Pap smear every 3 years
  • Colorectal — Adults 45–50+ — FOBT annually; colonoscopy every 10 years
  • Liver — People with hepatitis B/C or cirrhosis — Ultrasound every 6 months
  • Oral — All adults, especially tobacco/gutka users — Annual oral exam
  • Prostate — Men 50+ — Discuss with doctor

Where to get screened in Pakistan:

  • Lahore: Shaukat Khanum (SKMCH), INMOL, Jinnah Hospital, Services Hospital
  • Karachi: Aga Khan University Hospital, Liaquat National Hospital, KIRAN
  • Islamabad/Rawalpindi: PIMS, Shifa International Hospital
  • Most private diagnostic centres offer mammograms, Pap smears, and blood tests.

Barriers to screening in Pakistan:

  • Cost (though many government hospitals offer free or subsidised screening)
  • Fear and stigma — "I don't want to know"
  • Lack of awareness — many people don't know screening exists
  • Cultural barriers — especially for women's health

Key message: Screening is not for people who feel ill — it is for healthy people who want to stay healthy. If you are over 40, talk to your doctor about which cancer screening tests you need. Early detection saves lives. Do not wait for symptoms.
